In order to provide the best possible service, we have created several forms that you may find helpful before your appointment with us.
Below are the forms for you to fill out prior to your pet's visit with Animal Dental Clinic. Filling these out in advance helps our team prepare for your arrival. Please either fill out the online forms or download and bring with you the filled in copies on the day of your appointment. If you are not able to fill out the online version, please arrive 10 minutes early to your appointment for staff to upload the information.

Animal Dental Clinic offers our patient form(s) online so you can complete it in the convenience of your own home or office.
- If you do not already have AdobeReader® installed on your computer, Click Here to download.
- Download the necessary form(s), print it out and fill in the required information.
- Fax us your printed and completed form(s) or bring it with you to your appointment.
- You can submit forms online.
Blood Work Requirements: Please email to [email protected]:
- Up to 6 years require the following within 6 months: CBC with differential, chemistry panel of at least 15, and electrolytes.
- 7 to 9 years require the following within 3 months: CBC with differential, chemistry panel of at least 17, electrolytes, and T4
- 10 years and older require the following on the day of their procedure at Animal Dental Clinic: CBC with differential, chemistry panel of at least 17, electrolytes, and T4
- NOTE: Some breeds, sizes, and health concerns may change these timelines. Please consult a staff member for additional information.
- Please Note: It is your responsibility to provide us a copy of the blood work. However, if it has not been done within the above timeline we are happy to do it the morning of the appointment.
